What it does not do is physically break apart scar tissue, despite how often you hear that online. What muscle scraping is

Muscle scraping is the everyday name for IASTM, or instrument-assisted soft tissue mobilization. It grew out of Gua Sha, a scraping technique from traditional Chinese medicine, and in a modern clinic we use smooth stainless-steel tools with edges shaped to fit the contours of the body. A physical therapist glides the tool along a muscle, tendon, or the connective tissue around it to find and treat areas that feel stiff, tender, or restricted.

How it actually works

For years the popular explanation was that scraping breaks up scar tissue and “adhesions.” That story does not hold up. The pressure needed to physically break down scar tissue is far more than any handheld tool safely delivers. What the current research points to instead is a nervous-system effect. As the tool moves over your skin and connective tissue, it stimulates sensory receptors that send signals to your brain and spinal cord. That input can turn down pain and reduce the protective guarding that keeps an area feeling tight, which is why many people feel looser right after a session.

Scraping does briefly increase blood flow to the area, which is part of why the skin reddens. Whether that added circulation speeds up healing in people is not proven, so we count on the reliable benefits, less pain and easier movement, rather than promising faster tissue repair.

Does muscle scraping actually work?

Being straight with you here matters. The research on muscle scraping is mixed. The best current evidence suggests it can reduce pain in the short term, and it does not clearly beat other hands-on techniques or exercise. Large reviews rate the overall quality of the studies as low. So we do not treat it as a cure or a shortcut. We treat it as a helpful tool that, for the right person, makes movement more comfortable so the rest of your therapy works better.

How it helps you move better

When an area is painful and guarded, movement feels heavy and stiff and your form suffers. By easing pain and improving how the tissues glide, muscle scraping can help you reach more of your available range of motion in the moment. That short window matters because it lets you practice exercises with better form and less pain, and that loading is what builds lasting strength and stability.

Does it hurt? What a session feels like

You will feel firm pressure, and the tool can feel a bit “gritty” as it passes over tense or textured areas. It should not be excruciating. Your therapist usually applies a light lotion so the tool glides smoothly. Afterward, some redness or small bruise-like spots can show up. These are tiny surface capillaries reacting to the pressure, and mild spotting is normal. Heavy bruising is not the goal and means the pressure was too high, so always tell your therapist how it feels.

A quick note on tendinitis vs tendinosis

This one matters, because it changes the whole plan. Many people who think they have tendinitis actually have tendinosis, which is degeneration of the tendon rather than simple inflammation. That difference is why rest alone often fails to fix it. A degenerated tendon needs to be rebuilt with progressive loading, and that is where the real recovery happens. Muscle scraping supports that work by reducing pain, but the loading is what remodels the tendon.

Who it can help, and who should wait

Muscle scraping is often used for conditions like Achilles or other tendon pain, plantar fasciitis, tennis elbow, and neck or shoulder tightness. If you are recovering from surgery, it can have a place later in your rehab, but only once your surgeon has cleared you and the incision has fully healed. It is never used over a fresh wound. The honest guideline: it is an add-on, not a standalone fix.

How we use it at Wright PT

We do not use muscle scraping on its own. We use it to make the work that actually rebuilds tissue more tolerable. Here is what that looks like across our clinics.

At our Twin Falls clinic, a patient came in with debilitating Achilles pain. On assessment it was tendinosis, a damaged, degenerated tendon that often gets mistaken for tendonitis, not simple inflammation. Rather than rest and wait, the patient chose to rebuild the tendon with us. We combined IASTM with eccentric and functional training and pain management, and the tendon remodeled over time. The result was a full recovery and better athleticism than before the injury. The scraping was not what healed the tendon. The loading was. The scraping helped make that loading possible.

At our Meridian clinic, a patient with lateral epicondylosis, a degenerative form of tennis elbow, had reached the point where working was difficult. We used the same approach: address the pain, then progressively load and remodel the tendon over an initial 45-day block, followed by ongoing home exercises. The patient was able to work again.

At our Snake River Landing clinic, a patient with a stiff knee limited by an old injury worked on mobility using IASTM alongside other treatment. Over the course of care, their knee range of motion improved by about 15 percent, enough to change how the joint moved and felt day to day.

Every case is different, and these are individual results rather than a promise. What they share is the same principle: muscle scraping opens a window, and the strengthening and loading are what make the change last.
